The Commission reserves the right to demand verification of any relevant information prior to making determination on any application.
A Continuing Education Elective Course Application form must also be submitted for each course for which applicant seeks
approval. This webform includes up to three Continuing Education Elective Course Application forms.

If you are an applicant with an out-of-state business address or a sole proprietor with an out-of-state residence address you must submit a completed
"Consent to Service of Process and Pleadings" form (REC 1.03)
